HOW DO I BECOME A PATIENT?

DO I NEED TO PREPARE ANYTHING PRIOR TO MY VISIT?
Prior to your appointment, please fill out the New Patient Form located at the bottom of this page. If you wish to fill out the form using a computer, you will need Adobe Acrobat Reader (free software, click to download) on your device. At the very bottom of the New Patient Form on the last page, you will see the option to "Save as PDF" so that you can save and email us this file as an attachment. You may also choose to "Print" and bring the printout to our office on your appointment. This will save time and allow us to efficiently spend the fully allotted time that we have reserved for you.
If you have access to any previous dental records such as x-rays, we ask that you obtain a copy and send them to us prior to your visit. This will help us gather as much pre-information about your dental health history so that we can better serve you.

Here at Smile Aurora Dental, we value transparency-- especially when $ is involved. Thus, prior to your visit, we also kindly ask that you provide us with your dental insurance information (if you have one). This information will be used to find out your coverage benefits prior to your visit. We do this as a courtesy for you so that we can provide the best estimate of any potential out-of-pocket costs that may be needed for this visit as well as for any proposed future treatments.

WHAT CAN I EXPECT MY FIRST VISIT TO BE LIKE?
At Smile Aurora Dental, we will begin with a comprehensive examination of your teeth and gums in order to create a treatment plan that is customized for your unique oral needs. Your examination will include low radiation digital x-rays to detect decay between the teeth, as well as an oral cancer screening. Comprehensive x-rays are necessary for us to complete a thorough evaluation of your oral health. If your previous dentist has recently taken x-rays, you may request that they forward them to our office. If additional x-rays are necessary, they can be taken at our office.
After your exam, we will clean your teeth using both hand instruments and a piezo ultrasonic scaler to gently break up plaque. If a deep cleaning is recommended, your cleaning will be completed at another appointment, as this takes more time. A routine cleaning can be done prior to your deep cleaning, if you desire.
At the end of your treatment, our team will present to you the proposed treatment plan that you and Dr. Kim discussed. The treatment plan will consist of an estimation of your insurance's treatment coverage as well as any out-of-pocket costs. We will also divide the treatment plan into segments based on priority to help you plan out your next appointment.
As our appreciation to you, we would like to leave you with a small gift containing our favorite oral products (all personally hand picked by Dr. Kim) for you to try at home!
NOTE: All patients under the age of 18 must be accompanied by a parent or guardian.

We get it! We know the last thing you probably want to do is sit in a dental chair (no hurt feelings!). At Smile Aurora Dental, we continuously strive to make your whole experience as seamless and comfortable as possible. We will never pressure you to sign up for any treatment, but rather provide recommendations in a nonjudgmental manner.
We have created a warm and inviting environment that is combined with a friendly and caring staff. So take a sigh of relief, sink into our heated massage dental chair, put on our steam lavender eye mask and relaaaaaax.
Dislike all that dental noise and that overhead light glare? We have a solution for you! Put on our custom designed shades, wear our Bose noise-canceling headphones, and watch your favorite sitcom on Netflix or Hulu.

*We also have oral conscious sedation (a pill taken by mouth to help you relax) for our most anxious patients. Please contact our office beforehand if you are interested in this option.
DO YOU TAKE MY INSURANCE?
We work with medicaid and most PPO insurance plans. As a courtesy, our team will file all insurance claims for you. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated.

At Smile Aurora Dental, we want to make it as easy as possible for you to receive the best dental care possible. We offer the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.
The cost of your dental treatment will vary depending on your individual needs and treatment plan. For your convenience, we accept all credit cards and personal checks. We also offer an affordable financing plan called CareCredit.

No dental insurance? We have something for you and your family.

We believe everyone should have access to great dental care. That’s why we created Smile Savings Plan to help individuals and families without dental insurance. For an affordable annual fee (we have plans for singles, couples, and family), you and each member of your family will receive the preventive dental care you need, as well as discounts on treatment procedures in our office.
Once purchased, our annual Plan includes the following for no additional charge:
